Build your packet, one section at a time

1. Agree on the support that is welcome

Write down the person's preferred name, language, contact method and the help they want. Agree who may receive which information, when that permission should be reviewed, and how someone can ask to change it. Keep passwords, PINs and unrestricted access codes out of the packet.

2. Make a contact and responsibility map

List a primary family contact, a nearby person and a backup. Ask each person what they can actually do and when they are reachable. Keep clinician, pharmacy and service-provider contact details in a separate section so a family task is not confused with professional care.

3. Keep records current

Note where the current medicine list, appointment records, discharge instructions and insurance documents are held. Record the date and source of each update. A family member may organize the paperwork; questions about instructions or medicines go back to the treating clinician or pharmacist.

4. Make a handoff easy to follow

For each agreed task, record the action, owner, time window, backup, outcome and next step. A useful handoff says what still needs doing. An app status or tick on paper records someone's report; follow up directly if the situation is unclear.

5. Prepare for appointments and a return home

Keep a short question list, the provider's confirmed appointment details, the transport contact and the person collecting follow-up instructions together. Ask the treating team to explain anything the family cannot carry out or does not understand.

6. Store and share deliberately

Give copies only to people the older person has agreed should receive them. Tell recipients which copy is current, where the master is kept, and what to do with outdated copies. Avoid public links or group messages containing private medical records.
