Returning home can leave a family with paperwork, calls, transport and appointments to organize. Use the treating team's discharge instructions as your starting point, then turn the practical work into a shared list of responsibilities.
Ask the hospital team to explain the written plan in a way the older person and family can use. Keep unresolved questions together and identify the appropriate person to contact. The team should supply the clinical instructions; this guide helps you organize the work around them.

Confirm who is arranging the journey, who will be present at home, and who will call any provider the treating team has recommended.
Write down unclear instructions and direct them to the hospital team. Ask the team which contact to use and what to do if that contact is unavailable.

Keep the current clinician-confirmed list with the discharge paperwork. Ask the prescriber or pharmacist to resolve differences between that list and older records.
